By submitting this form you agree to the following conditions:

• I understand that my application is incomplete and considered null and void until full payment is received by Sacramento United. Once the fees are received a confirmation of my receipt of payment will be sent by Sacramento United. This confirmation of receipt of payment is not considered as acceptance into the tournament. I will be notified of acceptance after June 20, 2008.
   
• Applications submitted will be held for 2 weeks for payment. If after 2 weeks, your payment has not been received, your application will be discarded.
   
• The tournament committee reserves the right to combine or eliminate age groups and/or move teams up to the next age group dependent upon number of applicants.
   
• I understand that if my team is not accepted, the entry fee will be returned in full. Further, I understand that teams withdrawing from the tournament must send an email to sactovlr@hotmail.com   prior to June 20th, 2008 and will receive a refund. Teams that drop after the June 20th deadline will forfeit their entire application fee.
   
  
• I understand that there is a mandatory team credentials check in on Friday evening, August 8 from 6 - 9 pm. A team representative must check in the team at this time. Location: Sacramento Hilton Hotel.
